Abstract

|
Internuclear ophthalmoplegia (INO) is characterized by adduction weakness in the ipsilesional eye, and dissociated abducting nystagmus in the contralateral eye. INO results from a lesion involving the medial longitudinal fasciculus (MLF) in the dorsomedial pons or midbrain. Pseudo-abducens palsy refers to a limitation of abduction in the presence of an intact abducens nerve. Clinical features include adduction nystagmus of the contralateral eye, medial rectus spasm of the contralateral eye, preserved abduction with the vestibulo-ocular reflex, and absence of strabismus in the primary gaze. We report pseudo-abducens palsy and INO in two patients with acute thalamo-mesencephalic infarction.
